Breach Frequency

Figures below are drawn from HHS OCR's Breach Portal data as reported by HIPAA Journal's annual analysis, and from HHS's own Annual Report to Congress. The two sources differ slightly because the portal continues to be updated with delayed reports after HHS finalizes its report — both are cited below.

725

Breaches of 500+ records reported to OCR in 2024 — the third consecutive year above 700.

Source: HIPAA Journal, 2024 Healthcare Data Breach Report (Jan. 2025)
~275M

Individuals' records breached in 2024 — a 63.5% increase over 2023, driven largely by the Change Healthcare breach (~190M records).

Source: HIPAA Journal, 2024 Healthcare Data Breach Report
81.2%

Of 2024 breaches were hacking/IT incidents (589 of 725), affecting 259M+ records — by far the dominant breach type.

15.7% / 2.5% / 0.6%

Share of 2024 breaches from unauthorized access/disclosure, loss/theft, and improper disposal, respectively.

4,335

Median breach size in 2024, by records affected. 61% of all reported breaches involved fewer than 10,000 records — most breaches are small, even though headlines focus on the largest ones.

73% / 16% / 11%

Share of 2024 breaches reported by healthcare providers, business associates, and health plans, respectively.

HHS's own Annual Report to Congress for 2024 cites 663 breaches of 500+ records and approximately 242.9 million individuals affected as of its publication date — a snapshot taken before the Breach Portal's later updates. HHS Annual Report to Congress, 2024 (PDF).

Breach Cost

From IBM and the Ponemon Institute's 2025 Cost of a Data Breach Report.

$7.42M

Average cost of a U.S. healthcare data breach in 2025 — down from $9.77M in 2024, but still the most expensive industry studied.

279 days

Average time to identify and contain a healthcare data breach — longer than the cross-industry average, meaning more time for exposed data to be exploited before anyone notices.

$5.56M

Average cost of a breach in the U.S. financial sector in 2025 — the second most expensive industry, well behind healthcare.

$10.22M

Average cost of a data breach across all U.S. industries in 2025 — a record high, up from $9.36M in 2024.

$4.44M

Global average cost of a data breach across all industries and countries in 2025 — down about 9% from $4.88M in 2024.

Attack Types in Healthcare

From Verizon's 2025 Data Breach Investigations Report (DBIR), healthcare industry section.

1,710

Security incidents tracked at healthcare organizations in the 2025 DBIR dataset, with 1,542 confirmed data disclosures.

Source: Verizon 2025 DBIR, via HIPAA Journal
67% / 30% / 4%

Share of healthcare breach actors that were external, internal, and partner, respectively (plus 1% involving multiple actor types).

Source: Verizon 2025 DBIR, via HIPAA Journal
90%

Of healthcare attacks were financially motivated. 16% involved an espionage motive — a sharp jump from just 1% the year before.

System Intrusion

Overtook miscellaneous errors as the leading breach pattern in healthcare in 2024, as attacker-driven incidents rose and accidental-exposure incidents declined.

Source: Verizon 2025 DBIR, via TechTarget / HealthTech Security
44%

Of breaches across all industries Verizon studied involved ransomware — up sharply from 32% the year before. 64% of ransomware victims did not pay.

Cross-industry figure, not healthcare-specific — Verizon's healthcare snapshot does not break out a separate ransomware percentage.

Methodology & Sources

Every statistic on this page is drawn from one of four primary sources: the HHS OCR Breach Portal (via HIPAA Journal's annual analysis and HHS's own Annual Report to Congress), the IBM / Ponemon Institute 2025 Cost of a Data Breach Report, the Verizon 2025 Data Breach Investigations Report, and HHS OCR's enforcement data. Where the primary report itself wasn't directly citable, a reputable secondary source quoting the report's own figures is cited and labeled instead. This page is reviewed and updated quarterly; last updated October 26, 2026.
